Support Coordination

Support Coordination is a service offered under the NDIS (National Disability Insurance Scheme) to help participants understand and implement their NDIS plan. It involves working with a participant to make the most of their NDIS funding, ensuring they have access to the supports and services they need to achieve their goals.

24CARE Australia provides Support Coordination services to help participants navigate the complexities of the NDIS, connect with appropriate services, and ensure that their plans are effectively managed and tailored to their needs.

Support Levels We Provide

1. Level 1: Support Connection
Our team assists participants in building their ability to connect with informal, community, and funded supports, enabling them to achieve their goals.

2. Level 2: Coordination of Supports
This involves a higher level of assistance where our Support Coordinators help participants manage and coordinate their supports effectively. We work to ensure that services are delivered consistently and meet the participant's needs.

3. Level 3: Specialist Support Coordination
For participants with more complex needs, our Specialist Support Coordinators provide in-depth support to manage challenges, reduce barriers, and maintain continuity of support services.

Support Coordinators within 24CARE Australia would be responsible for:

1. Understanding Participants’ Needs: Working closely with participants to understand their specific needs, goals, and preferences.

2. Connecting with Services: Helping participants connect with the right services within 24CARE Australia, to ensure they receive the support they require.

3. Plan Implementation: Assisting participants in implementing their NDIS plans effectively, including budgeting and managing their funding.

4. Problem-Solving: Addressing any issues or challenges that arise in accessing services, advocating on behalf of participants, and ensuring that services are delivered in a timely and effective manner.

5. Monitoring and Reviewing: Regularly monitoring the participant’s progress and reviewing their plan to ensure it continues to meet their needs.
